Information this site collects

Forms may collect contact details, role, county, general service interest, message, employment interest, availability, and an optional resume. Do not submit diagnoses, health records, Social Security or Medicaid numbers, medication information, or support plans.

How information is used

Holley Star uses submissions to respond to inquiries, coordinate an initial referral conversation, or review an employment inquiry.

Holley Star Guide

The Holley Star Guide uses an external AI service to generate general informational responses from approved Holley Star and official APD information. Do not enter sensitive health or personal information. Guide interactions are not service intake. Question content and limited operational data are processed only as necessary to provide and protect the feature; this site does not create permanent visitor chat profiles or a transcript database.

Form delivery and retention

Production forms deliver submissions to a configured agency mailbox. Authorized staff should retain or delete records under applicable agency requirements. Uploads are not intended for permanent storage in the public web directory.

Cookies, analytics, and advertising

The site uses Google Analytics to understand general website usage and improve the website. Analytics is not configured to receive public-form contents, Guide questions, referral details, resumes, or other visitor-submitted sensitive information. Form and Guide security may use a short-lived essential session cookie. The site does not use advertising trackers.

Security and limits

Reasonable safeguards are used, but no internet transmission can be guaranteed completely secure. Use 911 for emergencies and approved secure channels for protected records.
