Which areas does Holley Star serve?
Holley Star currently serves the 11 counties in Florida’s APD Suncoast Region: Charlotte, Collier, DeSoto, Glades, Hendry, Hillsborough, Lee, Manatee, Pasco, Pinellas, and Sarasota.
Does Holley Star serve all of Florida?
Not currently. Holley Star serves the Suncoast Region and is preparing to expand into additional Florida regions.
What services does Holley Star provide?
Personal Supports; Life Skills Development Level 1 — Companion; Respite; and Supported Living Coaching.
How do I begin services?
Use Start Services or call Holley Star. We review general needs, requested service, county, availability, authorization, and service fit.
Can a family contact Holley Star directly?
Yes. A family may begin an initial conversation even when the exact service name is not known.
How can a WSC make a referral?
Use the Professional Referral form with basic contact, county, and service information. Protected records are exchanged later through an approved secure method.
What happens after I submit an inquiry?
The team reviews the inquiry, follows up appropriately, coordinates availability and authorization when applicable, and communicates next steps.
Does submitting a referral guarantee service?
No. Service depends on fit, authorization where required, agency acceptance, staffing, location, and current availability.
How does Holley Star support person-centered goals?
Staff align support with the person’s approved goals, routines, communication, preferences, informed choices, and desired participation in everyday life.
Can I upload a support plan or diagnosis?
No. Do not use public forms for protected records. Holley Star will identify an approved secure method if records are needed.
Are these nursing or medical services?
No. The four services described on this website are nonmedical supports.
How do I apply to work with Holley Star?
Use Careers to submit evergreen employment interest and an optional resume. Openings vary and are confirmed during follow-up.
